Lancashire Children's Multi agency Reviews Process and National Panel Reports

Our Review Process Pack provides a clear and consistent framework for how case reviews are initiated, managed, and delivered across the partnership. Its purpose is to ensure that all reviews are undertaken with transparency, fairness, and a strong focus on learning that improves outcomes for children, young people, and families. The pack brings together the key steps, responsibilities, and templates needed to support effective multi-agency practice.

The process begins with guidance on the criteria and pathways for initiating a case review, including how referrals should be submitted and what information is required. This ensures that decisions are made in a consistent and accountable way, and that agencies understand how notifications are considered by the relevant panels or oversight groups. Clear governance arrangements help ensure that communication with practitioners and families remains timely and respectful.

Once a case review is commissioned, the pack outlines the methodology, timeframes, and required documentation. It explains the roles of lead reviewers, agency representatives, and administrative support, alongside expectations for chronologies, analysis, and reflective learning. The pack provides structured templates to help partners contribute information that is concise, accurate, and focused on key learning points.

The Process Pack also sets out how findings are quality assured, written, and shared. It includes guidance on involving families, drafting reports, obtaining approval, and publishing learning where appropriate. The aim is to ensure that learning is not only identified but embedded across the partnership through training, briefings, and targeted improvement actions. By following this structured approach, the Case Review Process Pack supports a culture of continuous learning and stronger safeguarding practice.
